WCS nurses work at each school to help support the health needs of students.
If a child has a complex or chronic health condition, it’s important for the child’s family to communicate with the school nurse. Some examples would include severe allergies, diabetes, asthma, cardiac conditions, catheterizations, tube feedings, adrenal insufficiency, daily prescription medication, etc.
WCS Health Services Page
Health forms can be found on the WCS Health Services Page.
- A WCS Medication Authorization Form is required for the nurse to administer medication at school, both over-the-counter and prescription. Nurses do not administer medications without proper authorization.
- An immunization record or medical/religious exemption is required for each new student before the first day of attendance.
- Students entering seventh grade need an updated immunization certificate that includes the administration of Tdap booster or an exemption.
- Families moving from out of state may make an online request for records to be transferred by the health department. A Tennessee healthcare provider can also provide a Tennessee immunization certificate.
- WCS Illness Guidelines are available for reference on the WCS website to determine when a student should stay home due to illness.
